Grant Application Part One

Thank you for your interest in applying for the Jackson Foundation Grant.


Before completing the application, please read these instructions carefully. Incomplete applications will not be considered until all requirements have been met. Applications must be received no later than the deadline dates below to receive consideration by the Foundation's Co-trustees.

Step 1Download the Application Form and complete it according to the following instructions

Please type answers to each question within the space provided. Do not extend the response onto a separate sheet and do not use a type smaller than 10pt.

Step 2 – Obtain original signatures of Board Officer and Chief Executive Officer from your organization.

Original Signatures must be on the first page of the application. Scan and save the application in PDF format to prepare for upload.

Step 3 – Prepare the required supplementary documents as listed below

Once you have gathered these documents, save them in PDF format to prepare for upload.

  1. Copy of IRS 501 (c) (3) Tax Exemption Determination Letter
  2. One page introduction letter
  3. List of Officers and Board Members
  4. Current Financial Statement

Step 4Optional – Prepare a PDF of a descriptive brochure of your organization if available and relating to funding requests and/ or resumes of key staff.
